Cortisol is supposed to rise sharply in the morning, carry you through the day and fall by night. Chronic stress bends that curve: flat and low all day, spiking at bedtime, or crashing mid-afternoon. Each shape feels different and needs a different plan, but a single blood draw at 8 a.m. reduces all of them to one number that often reads "normal."

The DUTCH Complete collects several samples across one day at home, so your report shows the actual daily free-cortisol pattern. That is the difference between "your cortisol is fine" and "here is why you cannot sleep."

Cortisol is made in the adrenal glands and converted into cortisone, an inflammation hormone, in the kidneys. The more stressed you are, the more cortisol you make; the more inflamed you are, the more of it converts to cortisone. The DUTCH report has a section just for the adrenals that plots both against a "mountain range" of normal values. Ideally the two match and every point sits inside the range. When they do not, the pattern tells your doctor whether the problem is production, conversion or inflammation.

It also explains why stressed patients end up with hormone and thyroid symptoms. When the body spends its building blocks on cortisol and cortisone, there is little left for the sex hormones and thyroid hormones. Push the other way, with too much estrogen, and estrogens can mimic thyroid hormone and provoke thyroid autoantibodies.
